چکیده

Cardiac resynchronization therapy (CRT) markedly reduces morbidity and mortality in patients with heart failure and prolonged QRS duration. Landmark trials have included over 4000 patients based on their electrocardiogram. A few small, observational, non-randomized, single centre studies of short duration have suggested that echocardiographic measurement of mechanical dyssynchrony may better identify patients likely to benefit from CRT. We objectively review the meaning and measurement of electrical and mechanical dyssynchrony, the strengths and weaknesses of echocardiographic indices of dyssynchrony, and the controversial issue of predicting response to treatment. We conclude that proposals to alter current guidelines for patient selection, and include echocardiography, are misguided. Echocardiographic assessment will only become credible and applicable to clinical practice once used to select patients for large prospective randomized trials which show an improvement in clinical outcome.

Regions of latest mechanical contraction correspond to regions of latest electrical activation: an electro-mechanical coupling study in patients undergoing cardiac resynchronization therapy

Background Numerous studies have shown that placing the left ventricular (LV) pacing lead in the latest contracting region (mechanically dyssynchronous) or the latest activated region (electrically dyssynchronous) of the LV improves patient response to Cardiac Resynchronization Therapy (CRT). However, no studies have examined electrical and mechanical dyssynchrony in the same patients to examin...

Cardiac Resynchronization Therapy

Cardiac Resynchronization Therapy. Cardiac resynchronization therapy (CRT) is a recently developed approach to treat dilated heart failure with discoordinate contraction. Such dyssynchrony typically stems from electrical delay that then translates into mechanical delay between the septal and lateral walls. Over the past decade, many studies have examined the pathophysiology of cardiac dyssynchr...

Mechanical Dyssynchrony is Similar in Different Patterns of Left Bundle-Branch Block

BACKGROUND Left bundle-branch block (LBBB) and the presence of systolic dysfunction are the major indications for cardiac resynchronization therapy (CRT). Mechanical ventricular dyssynchrony on echocardiography can help identify patients responsive to CRT. Left bundle-branch block can have different morphologic patterns. OBJECTIVE To compare the prevalence of mechanical dyssynchrony in differ...

Relationship between MRI-based mechanical activation delays and direct intraoperative measurements of electrical delay in patients undergoing cardiac resynchronization

Background Magnetic Resonance Imaging (MRI) can be used to create maps of left ventricular (LV) mechanical activation delay by analyzing time-dependent radial motion of the myocardium. Local electrograms (EGM) can be obtained during the CRT procedure to measure local electrical activation delay in the LV myocardium. Preoperative MRI assessment of regional dyssynchrony would be useful to guide C...

Electromechanical and scar characteristics at left ventricular lead implant site in the context of overall dyssynchrony with cine DENSE predict cardiac resynchronization therapy outcomes

Background In clinical patients undergoing cardiac resynchronization therapy (CRT), myocardial properties (mechanical, electrical, and scar-mediated) at the left ventricular (LV) lead position (LVLP) and overall LV dyssynchrony (circumferential uniformity ratio estimate [CURE, 0-1]) are of interest with respect to CRT outcomes. We sought to define the relative importance of these factors for CR...
